Party Sign-Up Form

K4-12th Grades

Please indicate the parties you will help with for the 2008-2009 school term.  

Each party will include refreshments, punch, ice, cups, plates, napkins, and decorations for the tables.  

A chairperson will be in charge of calling parents who have signed up for that party.   Check the party or parties you would like to help with.
